Genesis Phase II

With the new "Genesis Phase II" Consumer Directed Service (CDS) Program your attendant can transport you to your doctor visits, shopping, bill paying & errands. You can direct your own service, you can hire your own care givers. Genesis Phase II serves as a vendor to provide the necessary administrative support services.

Consumer Directed Services is a Medicaid funded program. It enables physically disabled individuals to continue to live independently within their own home rather than going to a nursing facility.

CDS empowers the Consumer to remain in control of their lives by giving them the freedom to choose their own caregiver. The caregiver may be a friend or family member other than a spouse. This freedom of choice gives individuals peace of mind knowing that their personal care assistance and assistance with activities of daily living are being done by someone they know and trust.

Individuals receiving in home services through the Missouri Department of Health and Senior Services can switch to this self-directed services, if they meet eligibility requirements.

Consumer Eligibility Requirements:

  • Be Medicaid eligible
  • Meet the level of care requirement set by DHSS
  • Be capable of living independently with personal care assistance services as
  • determined by the state.
  • Be physically disabled.
  • Be at least 18 years of age.
  • Be fully able and willing to direct his/her own care.
  • NOTE: The personal care attendant cannot be the spouse of the consumer.

Attendant Eligibility Requirements

  • Be at least 18 years of age.
  • Have a clean background check through the Family Care Safety Registry or have an approved Good Cause Waiver.
  • Be able to meet the physical and mental demands required to perform specific tasks as required by the consumer.
  • Agree to maintain confidentiality.
  • Not the spouse of the consumer.

Consumer Responsibilities:

  • Selection, hiring, and training of the personal care attendant.
  • Schedule and supervise personal care attendants.
  • Prepare bi-weekly time sheets, ensuring they are accurate, legible, signed by both the consumer and the personal care attendant and submitted to Genesis Phase II in a timely manner.
  • Notify Genesis Phase II of any changes in circumstances, such as change in address or phone number.

Genesis Phase II Responsibilities:

  • Evaluation of new consumers to determine eligibility of the CDS program.
  • Preparing and submitting required paperwork to DHSS.
  • Provide training to the consumer on hiring and supervising personal care attendants.
  • Background checks on personal care attendants through the Family Care Safety Registry.
  • Payroll and billing processing on behalf of the consumer.
  • Ensuring all payroll, employment, and other taxes are paid in a timely manner on behalf of the consumer.
  • Perform monthly contact with each consumer to assure the consumer's care plan is meeting their needs.
